POSITION TITLE: Member Health Educator (Bilingual Tier I & Tier II)

Definition

Under the direction of the Kern Health Systems (KHS) Health Education Cultural & Linguistics Manager, the Member Health Educator will coordinate and promote activities related to health education, cultural linguistics and disease management. Education is defined as those activities that promote an enhanced understanding of access and utilization of KHS Plan services as well as increasing the knowledge of disease processes and disease control in members identified with the need. The Member Health Educator will coordinate the KHS Member Health Education and Cultural and Linguistics Programs following the guidelines and standards set down by the Department of Health Care Services (DHCS) and monitor the effectiveness of that program through Quality Improvement data collection and interpretation. The KHS Member Health Educator will assist in coordination efforts with the Quality Improvement Department to plan, implement, and promote a member disease management programs. The KHS Member Health Educator will participate in coordination efforts with KHS Provider Relations, Member Services, and Marketing to plan and implement an integrated orientation program and design written materials to inform and promote Plan services, cultural and linguistics and educational programs. This position will assist Provider Relations and the Health Education, Disease Management and Cultural & Linguistics Manager in maintaining the member education network composed of contracted providers, existing community programs and internal classes that will function to deliver a comprehensive Member Health Education Program to the membership of Kern Health Systems. This position will also assist, coordinate, and promote provider education, cultural competency and linguistic sensitivity through several avenues such as provider education forums, provider bulletins and/or provider visits.

Distinguishing Characteristics

This position is responsible for assigned Health Education, cultural and linguistics and disease management functions as defined by the KHS Health Education and Cultural & Linguistics Manager for a Knox-Keene licensed health maintenance organization (HMO).

Essential Functions

  • Assist in the design, implementation and maintenance of a comprehensive Health Education and Cultural and Linguistics Plan and Program for KHS that meets the requirements and standards of DHCS and NCQA.
  • Assist in the development and maintenance of a member health education network composed of contract providers, existing community programs and internalized programs.
  • Assist in the design and implementation of a provider orientation/education program that informs and inservices contract providers on the KHS Health Education and Cultural and Linguistic Programs.
  • Assist in the design, development and implementation of an integrated KHS member orientation program encompassing both access to Plan services, free interpretation and plan educational programs in collaboration with the KHS Member Services Department, the Provider Relations Department, and the Marketing Department.
  • Actively participate in the Public Policy/Community Advisory Committee by giving direction and input with information gathered from community groups, assessment tools, and strategic planning.
  • Assist nursing staff in finding and interpreting resources from the literature on specific programs.
  • Assist in the presentation of each program in a written format that will be used to define the programs.
  • Coordinate and the implement each program as far as arranging for provider and member education, arranging provider site visits by nurses, and design of appropriate data reporting methodologies.
  • Develop and complete the evaluation phase reports including appropriate data to assist executive staff in determining the effectiveness of each program in improving patient outcomes and reducing resource consumption.
  • Coordinate provider education lectures/forums and/or informational opportunities to present information on requested or identified health related topics.
  • Participates in collaboration efforts with the Kern County Health Department programs, e.g., Maternal, Child and Adolescent Health; Comprehensive Perinatal Services; Child Health and Disability Prevention; Acquired Immune Deficiency Syndrome; T.B. (D.O.T.) Program; and the Tobacco Control Program as well as the director of Health Promotion and the Public Health Officer of Kern County.
  • Provides individual health educational consultation (when requested) to the membership of KHS through established internalized programs.
  • Develop and plan distribution of written health educational materials such as articles, television or radio scripts, reports, and visual aids in collaboration with community resources and other KHS departments.
  • Addresses the community as a representative of KHS on pertinent health education or health care issues.
  • Participate in the development of surveys following DHCS guidelines to identify community and Plan educational needs and the effectiveness of implemented programs.
  • Coordinates the development and implementation of innovative tools to assist provider network and health plan staff in improving language and culturally appropriate care for member population.
  • Develops and strengthens linkages with community based organizations and educational institutions and foundations, in order to increase available resources for compliance, and to increase knowledge of member population and to improve service delivery.
  • Develops surveys which comply with state regulations to identify community and plan cultural and language needs and the effectiveness of the implemented program. Works in conjunction with the Health Education and Cultural & Linguistics Specialist in the needs assessment design, information collection, tabulation and information analysis in order to complete the Group Needs Assessment and assist in the delivery of culturally competent and linguistically appropriate programs and services.
  • Develops, implements and coordinates approved plans to ensure timely and effective communications. Focuses on educating members and providers on how to use linguistic services and appropriate cultural sensitive services.
  • Assists with monitoring systems to ensure compliance with State, Federal and contractual/regulatory requirements regarding Marketing and Cultural & Linguistic Services.
  • Assists with ensuring that tasks and projects related to interpretation and translation service requests have been performed within established policies, procedures and guidelines.
  • Assists with overseeing the translation of materials in all threshold languages and coordinates the evaluation of member, provider and marketing materials for cultural and linguistic appropriateness.
  • Assists with providing guidance on coordinating the translation process, assigning interpretation requests, creating and maintaining translation specifications and project glossaries, communicating with internal and external customers on linguistic issues, leading vendors, and answering questions.
  • Assists with providing guidance to coordinate the recruitment of qualified language vendors and consultants; manages contracted vendors, educating them on project expectations, workflow, timeline, quality processes and all other specifications, and coordinating cultural competency and linguistic review for languages other than English. Project quality is ensured through proofreading, formatting and follow-up with translators.
  • Assist in development of assessment tools in collaboration with DHCS to identify KHS members’ educational and cultural and linguistic needs and instruct PCPs in the use of such tools.

Employment Standards

Education: Possession of a Master's Degree in Public Health or related field required. Master’s Degree in Public Health preferred. Possession of a valid California Driver's license and State required auto liability insurance. Candidates are required to submit verification of their Master's Degree with their application.

Experience: Two years experience as a Health Educator with exposure to the design and structure of a comprehensive Member Health Education and Cultural and Linguistic Program. Experience in a public health and/or culturally diverse health care setting preferred. Bilingual Spanish/English. Certification required (Tier I & II verbal/written).

Possession of a valid California driver’s license and state required auto liability insurance; Required Travel Up to 40%.

Ability to: Plan, coordinate, implement and report all aspects of the Plan Health Education and Cultural and Linguistics program; prepare concise reports; communicate effectively interdepartmentally and with contract providers, provider physicians and community agencies; maintain proper documentation and confidentiality on member or Plan activities; publicly represent KHS at official functions.

Knowledge of: Quality Improvement programs for monitoring effectiveness of educational programs. NCQA and HEDIS standards and data collection, pertaining to Health Education, cultural and linguistics and disease management. Community based educational programs and other local resources.
