Nurse Manager, San Francisco, CA
The Nurse Manager is responsible for providing oversight and training for all paramedical and specialized training services for the frail, elderly, and people with disabilities served by Homebridge contract mode services.
This position works closely with other Service Delivery Managers in the San Francisco Home Care Services unit, but with specific responsibility for overseeing and providing supervision and leadership to the agency’s Nurse Case Manager and Specialized Training Coordinators. This position creates and maintains systems that ensure quality client assessments, care coordination, and home care provider training within Homebridge’s scope of practice and in accordance with industry best practices. This position oversees the quality and accuracy of client’s health assessments and collaborates with county partners and referring medical professionals to ensure high quality paramedical and specialized training service provision to Homebridge clients. This position also provides consultation, assessment, and health education services to Homebridge staff to ensure services delivered are high quality and in compliance with Homebridge’s contract provisions, policies and procedures, and within the scope of the agency’s resources and mission.
Education and Experience
Bachelor of Science degree in nursing preferred; must be a Registered Nurse (with education in public health, health education, health administration, gerontology, or equivalent), valid current license to practice nursing in the State of California and current CPR and First Aid certificate required. Three years’ experience, two of which are in public health, community social service or home health agency performing case assessments, intervention and developing care plans required. Two years’ experience supervising or managing a work unit. Experience in a nonprofit and working with the elderly and disabled communities desirable.
