Overview:

The Children’s Care Coordinator coordinates services and required documentation for children in Intensive Community-based Treatment Services (ICTS). The Children’s Care Coordinator works with families and youth to identify and articulate their primary needs and vision, and determine who they want to help them achieve their goals. Manages the team process to ensure the team mission and goals address the family’s priority needs. The Coordinator must be outgoing and able to engage youth and families, and at the same time be an effective listener. The Coordinator must be open to different cultures and values to actively support visions and goals that may be different than their own.

Responsibilities:

  • Coordinates between families, treatment agencies and community partners. Performs diagnostic assessments and formulates diagnoses.
  • Develops primary service plans in conjunction with the client and other individuals as indicated. Completes service plan and assessment updates.
  • Assesses symptoms and manages immediate crisis including danger to self or others, and implements crisis intervention plan if necessary.
  • Implements service planning goals, utilizing program-specific evidence-based practices
  • Develops primary service plans.
  • Organizes, schedules and facilitates Child and Family Team meetings.
  • Develops, writes and updates Care Coordination plans.

Qualifications:

Candidates must possess a Master's degree in psychology or a related field, an understanding of co-occurring disorders, and related experience. Licensure or ability to obtain Oregon licensure (LCSW, LMFT, LPC or Psychologist) within six (6) months a plus. Experience with Oregon children's mental health programs preferred. Demonstrated understanding of children's mental health levels of care, determination of such levels and experience working in systems of care.

Candidate must be able to work independently and flexibly, under general supervision. Computer proficiency required to use Electronic Health Records effectively Must pass state-required background check and DMV checks; possess a valid OR driver's license and demonstrate a safe driving record.
