POSITION DESCRIPTION


Responsible and accountable for the dialysis of chronic patients and the safe and efficient operation of the Dialysis Unit and staff. Responsible for delegating nursing and technician duties and assignments, coordinating schedules, monitoring admissions and discharges.


ESSENTIAL DUTIES & RESPONSIBILITIES



  • Ensures participation of Hemodialysis Services personnel in the formulation of the standards of nursing practices, policies and procedures.

  • Implements standards, annually reviewing and revising, as necessary, to reflect changes in nursing practice.

  • Support the cultural and religious preferences of the patients as long as such practices do not harm others or interfere with the planned course of medical therapy.

  • Demonstrate proficiency and supervise the documentation of the water treatment system and protocols.

  • Ensure that necessary labs are ordered, resulted, and redrawn if necessary.

  • Medication algorithm adjustments and appropriate communication with Medical Director

  • Fluid management strategies and evaluation.

  • Managing the schedule of both patients and staff.

  • Case management of all medical and psychosocial needs of the patient during admission, discharge and transfer.

  • Oversight of clinical staff duties including audits, training, and implementation of clinical improvement initiatives set forth by the Facility Administrator and/or Medical Director.

  • Compliance reporting through the National Healthcare Safety Network (NHSN) and Network 18.

  • Ensuring that Medical Director rounding is completed and all orders implemented within a timely manner.

  • Comprehensive Interdisciplinary Assessment (CIPA) and vaccination tracking and completion.

  • Provide clinical support to floor staff and cover missing shifts if necessary.

  • Perform other job-related duties as assigned.

  • Promotes teamwork and keeps a positive attitude at all times.

  • Listens to patient, family and physician concerns and takes their suggestions into consideration.

  • Welcomes and orients new patients to the facility.

  • Monitors patient schedules to ensure that staffing ratios are maintained for patient care and safety.

  • Assigns duties to staff in order to provide for optimum care while observing staff to ensure that policies, procedures, compliance, competency and appropriate regulations are being followed.

  • Participates in any emergency situation and communicates the conditions or results to appropriate team members.

  • Makes routine patient rounds to identify patient needs, assess quality of care and resolve issues.

  • Fills in as a staff team member, when needed.

  • Ensures that Confidential Event Reports are completed and appropriate action is taken.

  • Monitors condition of patient environment (physical plant) and is responsible for correction of any concerns.

  • Must conduct audits of the bio med technician areas of responsibility.  Safety testing for the water system, maintenance of machine/equipment repairs, quality testing and documentation.

  • Manages clinical staff on a day to day basis.

  • Prepares monthly staff schedule and ensures coverage for all shifts.

  • Evaluates the performance of clinical personnel and provides counseling.

  • Monitors employees for compliance with federal regulations, ESRD, OSHA, and other regulatory requirements.

  • Communicates, interprets, implements and is supportive of the policies and procedures of the dialysis facility.

  • Responsible for nursing QAQI outcomes data collection, submission to QAQI in a timely manner, and developing/implementing action plan for improvement.

  • Schedules and conducts Comprehensive Assessment and Plan of Care meetings with physicians and appropriate support staff in order to develop the individualized patient prescription.

  • Reviews monthly QAQI outcomes report with physician.

  • Responsible for maintaining medical staff files.

  • Involves physicians in education of staff.

  • Keeps Medical Director informed of events in the unit, and reviews all event reports for signature.


                                                                                                                                                                                                  


REQUIRED EDUCATION, EXPERIENCE, CERTIFICATIONS AND LICENSES:


 



  • Graduate from an accredited school of nursing and hold a current California R.N. license.

  • Minimum of 12 months of experience in dialysis nursing, and an additional 6 months of experience in providing nursing care to patients on outpatient dialysis.

  • Must be sensitive and possess an awareness and keen appreciation of Indian traditions, customs, and socioeconomic needs of the Indian community.

  • Cardiopulmonary resuscitation (CPR) certified or obtain within three months of hire.

PHYSICAL DEMANDS:


 


The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Sitting / Mobility: Approximately 20% of time is spent working at a desk. Balance of time (approximately 80%) is spent moving around work areas. Communication: Ability to effectively communicate with co-workers, patients and members of outside agencies, in person, by telephone, and by email. Vision: Ability to view x-ray, medical records, effectively use a computer screen and interpret printed materials, memos, and other appropriate paperwork. Lifting / Carrying: Ability to occasionally lift and/or move objects weighing up to 60 pounds. Stooping / Kneeling: Ability to provide patient care, access files/stock supplies in low cabinets and shelves. Reaching / Handling:  Ability to use hands to finger, handle, or feel; reach with hands and arms, input information into computer systems and retrieve and work with appropriate paperwork, equipment, and supplies. Use of standard medical instruments and equipment, including x-ray. Office equipment including, but not limited to computer, telephone, calculator, copiers, and fax. Work is performed in a medical office/clinic environment, with continuous contact with other staff and the public.
